Chemotherapy-induced nausea and vomiting (CINV) affects nearly 40% of patients undergoing cancer treatment and remains a major barrier to treatment adherence and adequate nutritional intake. In this prospective randomized study, we evaluated whether an oral nutritional supplement (ONS) containing ginger extract could alleviate CINV. Seventy-five patients were randomized to a control group without ONS, a conventional ONS group, or a fat-free ONS containing ginger extract. After the second cycle of chemotherapy, patients receiving the ginger-containing ONS exhibited markedly lower nausea scores compared with both the control and conventional ONS groups. This improvement was accompanied by a significant reduction in serum substance P levels, a key mediator of emesis. Notably, the ginger ONS group also showed increased energy and protein intake, along with improvements in body weight, lean body mass, and quality of life. These findings suggest that a specifically formulated ginger-containing ONS may represent a promising supportive strategy to mitigate CINV while preserving nutritional status during chemotherapy.
